The University of Arizona College of Pharmacy is committed to handling complaints in a fair and expeditious manner. Complaints may relate to general matters involving the curriculum, faculty, staff or students of the College. Some complaints are addressed by University policies, College Professionalism Code, or the specific standards established by the Accreditation Council on Pharmaceutical Education (ACPE).
- A concern/complaint against a student, staff or faculty member with regard to professionalism including code of conduct or academic integrity should be filed through the Professionalism Comment Form (please proceed to form below).
- A concern/complaint against College’s general operating procedures by a specific department or entity should be filed through Professionalism Comment Form (please proceed to form below).
- A concern/complaint against the College related to the ACPE standards policies or procedures shall be filed through ACPE only.
The Professionalism Comment Form provides a mechanism for pharmacy students, residents, fellows, faculty and staff to report to the Professionalism Committee either exemplary professional behavior OR lapses in professional behavior.
The purpose of the UA College of Pharmacy Professionalism Committee, which is composed of faculty, staff, and students, is to promote and reward excellence in professional behavior and address inappropriate conduct.
Submitting exemplary behavior provides an avenue to identify pharmacy students, residents, fellows, faculty or staff who have modeled behavior we would like to recognize. Submitting a comment about a lack of professionalism will start a process to address a concern, which may or may not result in disciplinary action against the individual about whom the comment was submitted.
While not completely anonymous, the College of Pharmacy will strive to maintain the privacy of the individual who submitted the comment. However, neither confidentiality nor anonymity cannot be guaranteed as sufficient information must be provided to allow the person to provide additional information if needed. If the identity of the person reporting is necessary, the individual about whom the comment was submitted will be warned that retaliation of any kind will not be tolerated and will be treated as a violation of the Professionalism Code.
The UA College of Pharmacy Professionalism Committee is only able to recognize or change behavior if comments are submitted.
If you experience or are aware of harassing or discriminatory conduct, you are urged to report the matter to the University’s Office of Institutional Equity. University policy strictly prohibits retaliation against an individual for reporting perceived harassment or discrimination or for participating in a resulting investigation.
