Maryam Fazel, PharmD, BCPS, BCACP, CDE, serves as assistant professor, Department of Pharmacy Practice and Science where she provides didactic education, experiential training, ambulatory clinical services and diabetes education. PGY-2 Ambulatory Care Pharmacy Residency Program Director and clinical pharmacist in diabetes care, Fazel enjoys a dual appointment at Department of Medicine, co-advising Diabetes Education, Prevention and Outreach Commitment to Underserved People program.

Board-certified as a pharmacotherapy specialist and ambulatory care pharmacist, she coordinates adult diabetes self-management education and support at Banner—University Medical Center South Campus. Her publications and many poster presentations address diabetes management and education, interprofessional practice, and medical education.
