The Parts of the Application
The College of Pharmacy application includes:
- Personal questionnaire
- Three online letters of recommendation
- Official transcripts of previous college-level coursework. (See application instructions for more details.)
- PCAT scores
- $65 nonrefundable application fee to be submitted within 10 days of submitting your application.
Applicants should send a check or money order payable to College of Pharmacy, The University of Arizona. Please write your application confirmation number on the check or money order and send the application fee and transcripts to:

Applicants are required to complete a University of Arizona Graduate Application and pay the application fee only if the applicant is admitted to the College of Pharmacy.
For admission to the Graduate School the student must have 72 units of college coursework completed by June 1 of the admission year.
