Medication Management Center - Phoenix 

Christine Aguilar, PharmD

Dr. Aguilar is a graduate of University of Illinois-Chicago College of Pharmacy, and has more than 20 years of clinical pharmacy experience.
 
Beginning her career in a large teaching hospital, Dr. Aguilar practiced both inpatient and outpatient pharmacy while providing training and mentoring to pharmacy students as a preceptor.  Dr. Aguilar then joined Abbott Laboratories where she created a Global Medical Information Center for the International Division.  She also worked as a Pharmacovigilance Specialist and product consultant for international opinion leaders, and as a product trainer for the international sales force, designing and delivering sessions in several countries.  More recently, Dr. Aguilar has worked in a variety of clinical settings, including an anti-coagulation clinic and a collaborative practice pilot program, both which served disadvantaged, mostly Spanish speaking patients, and with CVS Caremark as part of the Federal Employee Patient-Centered Care Team. 
 
As a Medication Therapy Management Specialist, Dr. Aguilar conducts one-on-one, comprehensive medication reviews for English and Spanish speaking patients to optimize the benefits of their medication regimens.  She enjoys helping patients understand their medications and manage their chronic medical conditions.  Dr. Aguilar also serves as a preceptor for College of Pharmacy students from the University of Arizona and Midwestern University.  
 
She also provides diabetes management services for Spanish speaking patients, as part of the SinfoniaRx collaboration with the Mexican consulate’s “Ventanillas de Salud” program.
 
Dr. Aguilar is a member of the Arizona Pharmacy Association, American Pharmacists Association, and the American Society for Medication Therapy Management.
 

Barbara Davee, PharmD 

Dr. Barbara Davee was born and raised in Puerto Rico. She moved to Tucson to attend the University of Arizona, where she completed her prerequisites and obtained a doctor of pharmacy at the College of Pharmacy in 2011. Dr. Davee went on to complete a postgraduate residency with Scottsdale Healthcare in Scottsdale, Arizona. She then practiced in the inpatient setting at Banner Baywood Medical Center in Mesa, Arizona for several years before joining the Medication Management Center Phoenix location as a medication management specialist in 2014. She currently helps lead the Medication Management Center's Spanish team in conducting medication reviews with Spanish-speaking patients nationwide. She also recruits and trains new student pharmacists for the Medication Management Center's year-round paid internship at the Phoenix location. Furthermore, she coordinates and precepts students during the Advanced Pharmacy Practice rotation offered at the MMC.

Anoiel Gevergizian, PharmD

Dr. Anoiel Gevergizian was born in Germany before moving to California at age six. He completed his undergraduate work at California State University Stanislaus in biological sciences. After working as a microbiologist for a year, he moved to Arizona for pharmacy school at Midwestern University. He began at the Medication Management Center as a second-year intern when the Phoenix location opened in 2014. Upon graduating from Midwestern University in 2016, he joined the team as a medication management specialist, conducting medication reviews with patients nationwide. 

 

Cameron Haun, PharmD

Dr. Cameron Haun was born and raised in Tucson, Arizona and attended the University of Arizona, where he completed both his Bachelor of Science in Microbiology and Doctorate of Pharmacy.  Dr. Haun began his career at the Medication Management Center (MMC) in 2015, after previously working in both the hospital and retail pharmacy settings.  Dr. Haun began working at the MMC’s Phoenix location and helped with the rapid expansion of the program. As a Medication Management Specialist, he helps with the training of student-pharmacists from the University of Arizona College of Pharmacy and Midwestern University and continues to precept them throughout the course of their internship or experiential rotation.  In his free time, Dr. Haun enjoys spending time with his wife (also a pharmacist, in pediatrics) and their beautiful baby girl and traveling to new places.

Patrick Walter Hryshko, PharmD

Dr. Patrick Hryshko grew up in Gilbert, Arizona and completed his pharmacy prerequisites at the University of Arizona in Tucson. After being admitted to the University of Arizona College of Pharmacy, he started working as a pharmacy intern for the Medication Management Center in 2011. He graduated with his doctorate degree in 2014 and continued with the MMC as a medication management specialist. He also coached patients enrolled in tobacco cessation programs. 

He relocated to the Phoenix center in 2015 and was appointed as a preceptor of pharmacy practice for the University of Arizona, where he teaches students how to assess disease state management and helps facilitate community health screenings. 

He is an active member of the American Pharmacists Association, Arizona Pharmacy Association, and the American Society of Medication Therapy Management. 

When he isn't educating patients and students on chronic conditions and medications, or volunteering with local community screening and outreach events, he spends his time traveling around the country as an ambassador for Kappa Psi Pharmaceutical Fraternity, Inc. 

Sally Lieu, PharmD 

Dr. Sally Lieu grew up in Sacramento, California. She attended the University of California, San Diego and earned her bachelor's degree in pharmacological chemistry. She went on to Midwestern University College of Pharmacy in Glendale and began as an intern at the Medication Management Center when it opened in Phoenix in 2014. After graduating with a doctor of pharmacy degree in 2016, she was hired as a medication management specialist to continue to provide medication therapy services to patients across the nation. 

 

James Montague, PharmD

Dr. James Montague is a native Arizonan, born in Phoenix. He completed his undergraduate course work at both University of Arizona and Arizona State University. After, he attended the Daniel K. Inouye College of Pharmacy in Hilo, Hawaii and was one of the founding classes, which helped establish the state's first fully accredited pharmacy school. Now he is a member of the Arizona Pharmacy Association and is the chair-elect for the Academy of Managed Care. Dr. Montague's mission is to further the field of pharmacy and maintain the high quality of care for patients which has made pharmacy one of the nation's most trusted professions. Currently he lives that mission at the Medication Management Center as a medication management specialist, where he completes one-on-one reviews with patients to optimize their medication regimens. 

Dawn Sullivan, PharmD, CGP 

Dr. Dawn Sullivan is a board-certified geriatric pharmacy specialist. She earned her doctor of pharmacy degree from the University of Arizona in 1995. Upon graduation, she then completed a pharmacy practice residency at the Southern Arizona VA Health System in Tucson. Dr. Sullivan has practiced pharmacy in a variety of settings including hospital acute care, anticoagulation therapy management, nursing home consultant, and ambulatory care clinical pharmacist at the Phoenix VA. She is currently practicing at the University of Arizona College of Pharmacy Medication Management Center in Phoenix. At the Medication Management Center, she provides medication therapy management services over the phone for patients acroos the United States with chronic conditions. Dr. Sullivan is also appointed as a clinical instructor in pharmacy practice and precepts pharmacy students. She serves as the personal pharmacist for the Sun Health Medication Management Program and provides comprehensive medication reviews for Gold Members at the Sun Health Community Wellness Center. She is also a member of the Arizona Pharmacy Association and the American Society of Consultant Pharmacists. In her spare time, she can be found at one of her teenage daughter's soccer games, track meets, or badminton games. She also enjoys boating, water skiing, cooking and spending time with her husband and daughters. 

Breanna Yano, PharmD

Dr. Breanna Yano was born and raised in Oregon, but moved to Phoenix, Arizona during high school. She attended the University of Arizona, completing her undergraduate degree in physiology, and attaining a doctor of pharmacy degree in 2015. In 2011, she began working at the Medication Management Center as a first-year intern. After graduation, she started full time with Target (now CVS) pharmacy, while continuing to work in per-diem for the MMC. In June of 2016, she began full time with the Medication Management Center and now works as a medication management specialist. Dr. Yano helps precept at the University of Arizona College of Pharmacy-Phoenix campus. In her free time, she enjoys traveling, exploring new restaurants, and spending time with friends and family.
