Goals
- To develop a pharmacist who can select, initiate, and monitor drug therapy in a manner to ensure the best outcome for the patient.
- To develop a pharmacist who can accurately and efficiently interpret media and literature to provide drug information in the practice setting.
- To develop a pharmacist who has good professional judgement in problem identification and problem-solving skills in patient care and other professional activities.
- To develop a pharmacist who has communication skills necessary for effective interpersonal/professional relationships.
Objectives
At the completion of this rotation the student should be able to:
- Obtain a history of medication use from patients.
- Retrieve, evaluate, and communicate in writing and verbally, specific drug information to health care professionals.
- Provide information in writing and verbally, about medication and devices to patients.
- Obtain the appropriate background information regarding signs and symptoms of the patients' illness and past and current drug therapy in order to select and/or recommend specific drug therapy.
- Communicate verbally and in writing specific recommendations regarding outcomes of drug therapy to patient and health care professionals.
- Utilize pharmacokinetic parameters in monitoring drug therapy.
- Utilize assessment parameters needed to monitor drug therapy and identify beneficial and adverse outcomes.
- Discuss the role of the pharmacist on the health care team.
- Effectively communicate with non-health care professionals.
- Identify patients who require clinical services.
- Prioritize the amount of clinical services required by patients.
- Prepare and give a formal presentation. To meet this objective, at least one presentation shall be given during each PhPr 810 rotation.
