Maryam Fazel, PharmD, BCPS, BCACP, CDE

Dr. Fazel is an assistant professor at the University of Arizona College of Pharmacy. She is a clincial pharmacy specialist in ambulatory care and an American Diabetes Association recognized Adult Diabetes Self-Management Education program coordinator at Banner - University Medical Center South Campus, Diabetes Physician Offices at Abrams and Diabetes Prevention and Education Center. She recently received a joint appointment in the College of Medicine. 

Dr. Fazel is a board-certified pharmacotherapy specialist, a board-certified ambulatory care pharmacist, and certified diabetes educator. Dr. Fazel practices as part of an interprofessional team that provides diabetes management and education. 

Dr. Fazel received her doctor of pharmacy degree from the UA College of Pharmacy. She completed a PGY1 pharmacy practice residency at the Southern Arizona Veteran Affairs Health Care System. 

Prior to joining the University of Arizona faculty, Dr. Fazel practiced in various areas of pharmacy including the community setting and as a specialist in poison information at the Arizona Poison and Drug Information Center. 

Dr. Fazel's research areas of interest are health outcomes, diabetes prevention/education, interprofessional practice and education.
